Create a QR Code for Microsoft Forms

In Microsoft Forms, use Collect responses, choose who can respond, and copy the response link. Do not use the browser address from the editing screen, a collaboration link, or a response-summary link. QRtracer opens the URL you supply; Microsoft hosts the form, enforces its audience setting, stores responses, and controls availability. If external visitors need access, confirm that the selected Microsoft Forms setting permits them before placing the code in public.

1. Publish the form and copy its respondent link from Collect responses.
2. Open the link in an incognito window or on a device without your Microsoft account.
3. Complete one test response and confirm that it appears in Forms.
4. Create the QR code and choose tracking if the destination may change.
5. Test the printed proof from the distance and lighting where people will scan.

For an event survey, name separate codes for the exit sign and printed program. Both may open the same form, while their scan histories preserve placement context.

A form limited to people in an organization can ask public scanners to sign in or deny access. An expired collection window or deleted form also creates a dead end even when the QR redirect works. Recheck the response link, audience, start and end settings, and confirmation message shortly before printing. If you use a dynamic QRtracer code, you can replace the destination with a new respondent link or a closed-survey explanation without changing the printed pattern.

A QRtracer scan means the form link opened; it does not mean a person answered or submitted. Use Microsoft Forms as the source of truth for response counts and response data.
